Do Thyroglobulin And Thyroid Peroxidase Antibodies Have A Pathogenic Role

Tg-Ab and TPO autoantibodies appear to be a secondary response to thyroid injury and are not thought to cause disease themselves, although they may contribute to its development and chronicity. Both types of antibodies are polyclonal, and although they are of the immunoglobulin G class, they are not restricted to one particular immunoglobulin G subclass. Polyclonality also mitigates against a primary role in disease pathogenesis.214,215 For example, these thyroid antibodies cannot transfer disease from mother to fetus or between animals even though they can pass across the placenta. However, both antibodies may contribute to disease mechanisms. For example, TPO-Ab on the surface of B cells may be involved in antigen presentation, thus activating thyroid-specific T cells.216 Such autoantibodies may have complement-fixing cytotoxic activity. And TPO-Ab, in particular, correlates well with thyroid damage and lymphocytic infiltration.

Immunologic Markers In Diagnosis

TG and TPO antibodies provide readily available evidence for Graves’ disease although not specific, their presence in a patient with hyperthyroidism strongly suggests the diagnosis. TSAb are not generally measured because present bioassays are laborious. TBII estimation is a commercially available surrogate but these antibodies are absent in some newly diagnosed patients and present in AT patients who have TSH-R blocking antibodies. TSAb or TBII levels have been investigated as predictive markers for the success of antithyroid drug treatment in Graves’ disease. Although patients with the highest levels tend to relapse the most, the results are insufficiently accurate for clinical use . The most important indication for TSAb assay in Graves’ disease is during pregnancy to predict the risk of neonatal thyrotoxicosis .

What does it mean if you have thyroid peroxidase antibodies floating around in your blood?

Well, it can mean several things:

  • It may be a marker for the autoimmune disease known as Hashimotos thyroiditis.
  • It may also be a marker for another autoimmune disease of the thyroid gland known as Graves disease .
  • It may be a transient thing that may eventually go away on its own ).

The presence of thyroid peroxidase antibodies is never normal but it doesnt always guarantee a poor outcome.

For instance, as I mentioned above, some people see elevated antibodies during pregnancy which then fall after birth.

The significance of thyroid peroxidase in this exact setting is not well understood but it does give some hope that not every situation of elevated antibodies points to autoimmune disease.

How Often Should You Test Your Thyroid Antibodies

I often hear from readers who say that their doctors refuse to retest their thyroid antibodies, stating that once a person is thyroid antibody positive, they will always be positive, so retesting the antibodies is useless. I disagree. Tracking your thyroid antibodies can give you an idea of how aggressive the attack is on your thyroid gland. While other lab markers and symptoms should be considered, generally, a lower thyroid antibody number indicates that your condition is improving, while a higher antibody number may indicate that you are having a flare-up or that your condition is worsening.

Tracking antibodies can give you an idea of whether the interventions youre utilizing are working, i.e. is your change in diet leading to an improvement in your condition?

When implementing active interventions, you can check antibody levels every 1-3 months to see a trend however, it will take anywhere from 3 months to 2 years to see the full impact of an intervention.

The correlation between the absolute number of T cell subsets and thyroid antibodies in pemphigus patients were also investigated in 58 PV patients and 34 PF patients for whom the T cell subset counts in peripheral blood were available. It was very interesting to find abnormal low CD3 + T cell counts were only present in patients whose thyroid antibody levels were less than 20 IU/ml, although with no statistical significance . Indeed, TPO antibodies are frequently mentioned in terms of thyroid health and diseases that affect it. Hypothyroidism Triiodothyronine And Thyroxin

Hypothyroidism can be a cause of infertility and, if untreated, may lead to increased complications during pregnancy, e.g. to more spontaneous abortions, hypertension of pregnancy, placenta previa or placenta abruptio . In populations with a sufficient iodine supply, chronic thyroiditis is the most common cause of thyroid hypoactivity. Also thyroid antibodies, without manifest hypothyroidism, increase the risk of spontaneous abortions and premature births . This was also confirmed by the results of a meta-analysis . Levothyroxine seems to lower this risk. Depending on the severity and duration of hypothyroidism during pregnancy, there may be impairment of neuropsychological development of the child. This is known, in particular in connection with iodine deficiency . Pop studied the developmental outcome of 63 children born to mothers with low free thyroxin during pregnancy compared to 62 children born to euthyroid mothers and found on the Bayley developmental scales an average reduction of 10 points on the mental scales, and of 8 points on the motor scales in the first and second year of life. Similarly, Li found delayed development at 2530 months of age in offspring of women with subclinical hypothyroidism, hypothyroxinemia or elevated levels of thyroid peroxidase antibodies. In a study from The Netherlands maternal hypothyroxinemia was found to be a significant risk factor in the offspring for language and cognitive delay at 30 months.

Stage : Subclinical Hypothyroidism Occurs

Stage 3 is when subclinical hypothyroidism occurs, and in this stage, people are going to have an elevated TSH in addition to having thyroid antibodies. They will also have immune cell infiltration into the thyroid gland, although their T3 and T4 levels are still going to be normal. Many people will have some symptoms of hypothyroidism or autoimmune symptoms at this stage.

In summary, high thyroglobulin antibody levels likely hold little clinical relevance to the presence and severity of thyroid disease. TPO antibodies are a greater indicator of thyroid health and can be used to monitor the progression of thyroid dysfunction.

However, even TPO antibodies come with their own limitations. Unless your TPO antibodies are very high, its not necessary to be hyper-focused on reducing your thyroid antibodies to the lab reference range. Reducing elevated TPO thyroid antibodies to below 500 IU/mL, and improving your thyroid function with diet and lifestyle changes significantly reduces your risk of developing hypothyroidism.

    The quality of selected articles was assessed independently by two authors by using the Newcastle-Ottawa-Scale . Each article was scored based on three dimensions: subject selection comparability and exposure . The total NOS score range from 0 to 8.

    Data Collection And Laboratory Measurements

    In order to accurately obtain clinical data during medical examination, interviewers were trained specially. Body weight and height were measured with an automatic body composition analyzer and BMI was calculated. Systolic blood pressure and diastolic blood pressure were recorded at rest.

    A fasting blood sample was collected. HbA1c, triglycerides , and high-density lipoprotein cholesterol levels were also measured using standard procedures at SRL, Inc. HbA1c and TG levels were measured by the enzyme method while the HDLc level was measured by the direct method. TSH, free T3, free T4, and TPO-Ab were measured by standard procedures at the LSI Medience Corporation TSH, free T3, and free T4 were measured by chemiluminescence immunoassay, and TPO-Ab was measured by electrochemiluminescence immunoassay.

    A normal range of TPO-Ab was defined as below 16IU/mL. Maximum values for the left and right common carotid arteries of the CIMT were calculated with an semiautomated digital edge-detection software following a protocol that has been described in detail elsewhere. The higher values of the right and left CIMT not including plaque measurements were then calculated, and the maximum CIMT value was then used for analysis. Since a previous study reported the normal CIMT value as < 1.1 mm, we defined atherosclerosis as CIMT 1.1 mm.

    Correlations Between Thyroid Indicators And Visceral Fat Area

    To further assess the relationship between thyroid diseases and visceral obesity, the correlation between VFA and TPOAb titer was analyzed using both Pearsons correlation analysis and linear correlation analysis. In this study, TSH, FT3 and FT4 levels were within the normal reference range for all participants in both groups. Pearsons correlation analysis suggested that the increased TPOAb titer was significantly positively correlated with VFA and SFA . Besides, we can see from Table 3, FT3 and TSH levels were also significantly positively correlated with VFA . Therefore, when we adjusted for the effect of TSH, the increased TPOAb titer was still significantly associated with visceral fat area. Linear correlation analysis also suggested the same results .

    Certain Conditions Can Make Your Thyroglobulin Antibody High

    Even though antithyroglobulin antibodies are not the best predictor of thyroid disorders, they should not necessarily be ignored. Unlike TPO antibodies, thyroglobulin antibodies are not able to stimulate an immune response in the thyroid, but they may be useful in diagnosing certain thyroid conditions .

    Anti-TG antibody levels may show importance in the following:

    • Patients diagnosed with thyroid carcinoma who have elevated TG antibodies may be at a higher risk of the cancer spreading to lymph nodes or surrounding organs. Elevated levels may also indicate a high risk of thyroid cancer recurrence after treatment .
    • Breast cancer patients may also have high thyroglobulin antibody test results .
    • Elevated TG-antibodies may have a higher prevalence in those with autoimmune connective tissue disease. This suggests that screening for thyroid disease in these populations may be beneficial .

    One study found that after successfully treating thyroid disease, thyroglobulin levels dropped back down into normal reference ranges . It is important to point out that this study tested the actual levels of thyroglobulin protein, not the TG-antibody levels.

    However, this finding is consistent with another study that found elevated anti-TG antibodies to correlate with the severity of hypothyroid symptoms, including fragile hair, swelling of the face and eyes, and vocal hoarseness .
