What Are The Clinical Features Of Hypothyroidism
Hypothyroidism results in a reduction in the bodys metabolic rate. It is characterised by:
- Cold peripheries with pale and dry coarse skin
- Eczema craquelé a form of dermatitis in which there is a crazy paving splitting of the skin surface
- A yellowish hue to the skin secondary to carotenoderma
- Sparse and brittle hair, which comes out in handfuls
- Loss of hair in the outer third of eyebrows
- Slow growing, ridged and brittle nails
- Myxoedema, resulting in puffy face, eyelids, hands and feet a presentation of mucinosis
Other common systemic symptoms due to hypothyroidism include weight gain, cold intolerance, low mood and menstrual disturbances .

Clinical features of congenital hypothyroidism include:
- Hypotonia and lethargy
- Feeding difficulties, resulting in poor weight gain and failure to thrive
- Hoarse cry with reduced frequency of crying

- Cold and mottled skin especially on the extremities
- Flattened or broadened nose associated with nasal obstruction

What Do Thyroid Rashes Look Like
Thyroid rashes can take on several appearances, but puffy, shiny skin that is red or discolored is common. Skin can also appear dull, flaky, dry, or rough, depending on your level of thyroid dysfunction.
Your healthcare provider may suspect a thyroid disease or a lack of control of your thyroid condition based on the appearance of your skin, but blood testing is necessary to confirm a diagnosis or guide treatment.
Autoimmune Disease And Chronic Hives: What Explains The Connection
The relationship between Hashimotos and chronic hives has been investigated and observed almost since the 1980s, says Betul Hatipoglu, MD, professor of medicine at Case Western Reserve University School of Medicine in Ohio. But its still not entirely clear exactly how or why they are connected.
Sometimes chronic hives develop years after a person is diagnosed with Hashimotos, Dr. Hatipoglu notes other times, a person first develops chronic hives and later is diagnosed with Hashimotos.
Scientists have discovered that chronic hives and Hashimotos have similarities, including elevated levels of immune chemicals that activate the immune system and lower levels of cells that suppress immune reactions , compared to a person with a healthy immune system.
Furthermore, blood tests show that both people with chronic hives and those with Hashimotos disease have autoantibodies against the thyroid gland, also known as antithyroid antibodies. One antibody common to both hives and Hashimotos is the thyroid peroxidase antibody, or TPO.
Another theory is that hives in Hashimotos stem from increased levels of a key hormone involved in normal thyroid activity, , which bears some molecular similarities to pro-inflammatory cytokines, a type of immune chemical.

Signs And Symptoms Of Hypothyroidism
Thyroid disorders are common. In fact, about 12% of people will experience abnormal thyroid function at some point during their lives.
Women are eight times more likely to develop a thyroid disorder than men. Also, thyroid problems increase with age and may affect adults differently than children.
At the most basic level, thyroid hormone is responsible for coordinating energy, growth and metabolism in your body.
Problems can occur when this hormones levels are too high or low.
Hypothyroidism, or low levels of thyroid hormone, slows your metabolism and decreases growth or repair of many parts of the body.
Cause Of Hives With Thyroid Disease
Autoimmunity is thought to be one of the most common causes of chronic hives.
Autoimmune diseases are those in which the body’s immune system mistakenly attacks its own cells and tissues. It does so with immune proteins called autoantibodies that cause chronic or recurrent attacks known as flares.
In the case of autoimmune thyroid disease, the thyroid gland is the target of the assault.
During an autoimmune flare, the spontaneous release of autoantibodies can trigger the release of another protein known as immunoglobulin E . IgE is the antibody responsible for allergic reactions, such as hives or rash.
Not all autoimmune diseases trigger an IgE response. Those that do produce autoantibodies that target a specific proteinâcalled a high-affinity IgE receptorâfound in tissues under the skin. The autoantibodies “turn on” these receptors, triggering the production of IgE and the chain of events that lead to allergies.
Hashimoto’s thyroiditis is one of several autoimmune diseases closely linked to chronic urticaria. Others include lupus and rheumatoid arthritis .
Chronic hives may occur in tandem with an autoimmune flare or when the disease is subclinical . During subclinical episodes, even low levels of autoantibodies can trigger an IgE response.
While Hashimoto’s thyroiditis may increase the risk of chronic hives, not everyone who has Hashimoto’s will get hives. This suggests that other environmental or genetic factors may contribute to the risk.

Hypothyroidism is diagnosed with thyroid function tests .
- Serum TSH is usually high.
- In pituitary or hypothalamic disease, TSH can be low or normal.
- Serum T4 and T3 levels are usually low.
- In subclinical hypothyroidism, TSH is mildly raised in the presence of normal T4 and T3.
- In sick euthyroid syndrome*, TSH, T4 and T3 are all low.
*Sick euthyroid syndrome commonly occurs in patients who suffer from severe non-thyroidal illness such as sepsis, burns and trauma. It is usually transient, and treatment may not be required, as thyroid function typically reverts to normal following disease recovery.

Serum autoantibodies are present in 9095% of autoimmune thyroiditis. They should include:
- Anti-thyroid peroxidase antibodies
- Anti-thyroglobulin antibodies.
Full blood count and inflammatory markers, such as C-reactive protein, are routinely included to screen for anaemia and systemic infection causing thyroiditis. Creatine kinase is tested to check for myopathy, and lipids are evaluated as hypercholesterolaemia and hypertriglyceridaemia are common in hypothyroidism.
Imaging should include:
- Ultrasound of the thyroid gland especially where hypothyroidism is associated with a goitre
Hypothyroidism: General Skin Findings
Hypothyroidism is most common in women 60 and older, but can affect both sexes at any age.6 Symptoms vary based on the severity of the hormone deficiency and on whether the hypothyroidism is the result of an autoimmune disease or occurs secondary to a nonautoimmune disorder, with autoimmune etiology being most strongly associated with skin manifestations.5
Common cutaneous findings in patients with hypothyroidism include rough dry skin with fine scales, most often on the extensor extremities.3 Skin might also be cold and pale with a yellow hue to the palms, soles, and nasolabial folds due to increased carotene in the dermis.2 In severe cases, patients can develop generalized myxedema , an edema-like skin condition caused by increased glycosaminoglycan deposition in the skin.2 In these cases, skin appears swollen, dry, and waxy, and there is easy bruising and poor wound healing. Other Causes Of Hyperthyroidism
Hyperthyroidism can also result from thyroid nodules. These are lumps that develop inside the thyroid and sometimes begin producing thyroid hormones. Large lumps may create a noticeable goiter. Smaller lumps can be detected with ultrasound. A thyroid uptake and scan can tell if the lump is producing too much thyroid hormone.
What Are The Types Of Thyroiditis
Types of thyroiditis include:
- Hashimotos thyroiditis: This autoimmune condition, also called chronic lymphocytic thyroiditis, is caused by antithyroid antibodies. Its the most common form of thyroiditis and the most common cause of hypothyroidism.
- Silent or painless thyroiditis: This is an autoimmune condition caused by antithyroid antibodies.
- Postpartum thyroiditis: This is an autoimmune condition caused by antithyroid antibodies that can occur within one year after giving birth. Its relatively rare.
- Radiation-induced thyroiditis: This is a condition caused by radiation therapy used as a medical treatment for certain cancers or by radioactive iodine used to treat hyperthyroidism.
- Subacute thyroiditis or de Quervains thyroiditis: This is an often painful condition thought to be caused by a virus. Its usually preceded by upper respiratory infections.
- Acute infectious thyroiditis: This is a rare condition caused by an infectious organism or bacterium.
- Drug-induced thyroiditis: This is a condition caused by the use of medications such as amiodarone, interferons, lithium and cytokines. It only occurs in a small fraction of people using these drugs.
- Riedel thyroiditis: This is a rare disease caused by chronic inflammation and fibrosis of your thyroid gland. Fibrosis is the thickening or scarring of tissue.

Skin Conditions Caused By Or Associated With Hashimotos Thyroiditis
It may be depressing to hear about all of the things that can go wrong with your skin if you have Hashimotos but I dont want you to feel that away and heres why:
Many of these skin conditions can be treated or at least dramatically improved by simply focusing on thyroid function.
If you can treat and improve your thyroid , you will often see significant improvement in your skin without the need for lotions, topical medications, or other treatments.

Dry skin is one of the most common symptoms of low thyroid which can occur in hypothyroidism as well as Hashimotos thyroiditis.
It occurs in as much as 65% of patients with Hashimotos thyroiditis which means that about 6 out of 10 people reading this may experience this symptom.
The medical term for dry skin is xerosis and the type of dry skin that occurs in low thyroid states is different from that seen in other conditions.
Usually, this type of dry skin is seen on the extensor surfaces of the body which means on the forearms, back of your hands, your back, and the back of your legs and shins.
