Treatment of thyroid carcinoma is usually dictated by the size of the mass, the extent of invasion and the presence or absence of gross metastatic disease.
Tumor volume has been correlated with metastatic potential . Tumors with a volume of less than 20 cm3 and no evidence of vascular invasion have a lower metastatic potential and follow-up chemotherapy is not routinely recommended. In contrast, tumors with a larger volume or evidence of vascular invasion have a higher metastatic rate and follow-up chemotherapy is recommended to prevent or slow the development of metastatic disease.
If tumors are freely moveable on physical examination, surgical removal is recommended. If surgical margins are free of disease and tumors are < 20 ccs in volume, further therapy is not advised. If tumors are > 100 ccs, adjuvant chemotherapy is advised. Between 20-100 ccs, systemic treatment recommendations vary. If surgical margins are not free of tumor cells, then adjuvant radiation therapy +/- chemotherapy is considered.
If tumors are fixed or invasive, preoperative radiation therapy followed by surgical excision if the mass responds to radiation is recommended. In cases where no more surgery is possible, chemotherapy is then advised.We have also palliatively used thyroid hormone supplementation in dogs with normal thyroid levels to suppress the production of TSH, thought to stimulate tumor growth.

What Does The Thyroid Do For Dogs

Radiation therapy
Radiation therapy consists of either conventional finely fractionated radiation therapy, palliative conventional radiation therapy and stereotactic radiation therapy , which are administered to dogs under general anaesthesia.
Conventional finely fractionated radiation therapy is typically administered daily, Monday to Friday for around 3.5 weeks. The median survival time reported with this approach is 2-4 years with the maximum response seen around 8-22 months after finishing radiation therapy.
Palliative conventional radiation therapy is typically administered weekly for four treatments. The median survival time reported with this approach is around six months to 2.4 years.
Stereotactic radiation therapy is a relatively new form of radiation therapy in veterinary medicine and typically involves three consecutive daily high doses of radiation therapy. In one recent study of 23 dogs with predominately advanced stage thyroid carcinoma, the overall response rate was 70%, with 81% of dogs experiencing clinical improvement at a median time of two weeks after starting SRT. The overall median survival time was around one year and for dogs with or without metastasis. SRT provides the benefit of a reduced number of general anaesthesias and shorter treatment duration. However, it is expensive.

Most thyroid cancers are very curable. In fact, the most common types of thyroid cancer papillary and follicular cancers have a more than 98% cure rate if theyre caught and treated at an early stage. The earlier you are diagnosed, the less likely it is that your cancer will have spread beyond the thyroid and the easier it is to treat.

Hepatocellular carcinoma can present in three ways:

Radiation is recommended for dogs that are not candidates for surgery and dogs that have undergone surgery but show signs of severe vascular invasion.

Recovery And Management Of Thyroid Cancer In Dogs
While there are risks with any anesthetic and surgical procedure, most dogs do well post-operatively after thyroidectomies and have no major complications. Recovery may require overnight monitoring in the hospital, with a next day discharge, in most cases.
Prognosis and life expectancy depend on the size and spread of the tumor. For thyroid tumors that are completely surgically removed, the median survival time can be upwards of two years. For tumors that require followup with chemotherapy and/or radiation, the prognosis is still good, with a median survival time ranging from 18 to 24 months. For dogs undergoing radiation and chemotherapy without surgical intervention, prognosis is poor, with an average survival time of between 6 and 12 months depending on the metastasis.

Hypothyroidism In Dogs Treatment
Treatment in dogs with hypothyroidism first requires diagnosis to help your vet understand what might be causing your dog’s symptoms.
